Registration Next Steps

Once you have received a placement notice from MPS, the next step is to register with the school!

Complete the Online Registration Form and bring the following documents to the school: 

• Birth Record: hospital record of birth, birth certificate, passport, resident alien card, or I-94
• Record of Immunizations
  • This Immunization Form lists required vaccines and includes the exemption form. Medical exemptions must be signed by a health care provider. Conscientious exemptions must be signed by the parent/guardian and notarized. 
• Early Childhood Screening Summary for children entering Kindergarten 
  • Book a screening appointment
• Special Education Records: Evaluation and current Individual Education Plan (IEP)
